Clinically significant disturbance in cognition, emotion regulation, or behaviorb. Significant distress/disabilityc. Not expectable or culturald. Not a society or individual conflict (e.g. political, religious, sexual, etc.)II. Diagnostic and statistical manual of mental disorders – DSM 5, fifth editiona. The second edition was very vague and had many things that are not considered mental disorders today:i. Ex. caffeine withdrawalii. Specific learning disorder; dyslexia, etc.iii. Premature ejactulationiv. Excessive drinkingv. HomosexualityIII. Case study #2 Derricka. It was during the time in between Christmas and New Yearsb. Dr. Halgin’s receptionist calls him and says that there is a young man sitting in the officei. The young man is very disheveledii. He isn’t wearing a jacketiii. His hair is wetiv. He hasn’t shavedv. He is shakingc. Dr. Halgin begins to ask himself certain questions…i. Does he have problems regulating his temperature?ii. What was the precipitant to this occurance?1. Precipitant – events leading up to Derrick’s current symptoms being displayed2. What led this person to come in?3. Is this his first time having these symptoms?4. Is he in danger?d. Dr. Halgin comes into officei. Asks himself, how involved do I want to get with this man?These notes represent a detailed interpretation of the professor’s lecture. Dr. Halgin lets Derrick come into the officei. Derrick is crying and shakingii. Derrick says, “My brain is going to explode. I haven’t slept in days. I’ve been going crazy since December 10th (15 days approximately). There is a stalker outside who’s been trying to kill me, and the police are in on it. I can’t stop washing, I’ve been taking 5-6 showers per day for the past few weeks.”IV. Dr. Halgin’s potential questions:a. Is he paranoid?b. Is there really a man stalking him?i. Is this possible?c. Is he psychotic?d. Is it a hallucination?e. What happened on December 10th? This may have been the precipitantf. Is he on medications or drugs?g. Why is he having these compulsions to shower?V. Dr. Halgin asks, “Derrick, what’s been going on with you?”a. Derrick seems out of controlb. When dealing with someone who is out of control, sometimes all they want is for someone to control them.VI. Derrick’s life story:a. He was accepted to an Ivy League but decided to go to UMass insteadb. Derrick was stressed out by the competition at the Ivy Leaguec. Everything got crazy at Thanksgivingd. Holidays are often a big day for therapists (Christmas, New Years, Mother’s & Father’s day)e. He got in a big fight with his father on Thanksgivingi. His father said that Derrick was a huge disappointmentii. There were a lot of mean words exchangediii. Derrick’s father had a heart attack and was sent to the hospitaliv. Derrick didn’t want to visit him because of the fight they got inVII. After this event, Derrick began to really freak out.a. He moved out of home and was couch surfing.b. He wasn’t taking good care of himselfc. On December 10th, Derrick went to the home of one of his openly gay high school teachers and told him that he wanted to have sex with him.d. His teacher said no, and was very confused about what was going on with Derrick.e. Derrick ran away screaming.f. From there, Derrick went into a brief psychotic disorderVIII. At this point, Dr. Halgin needs to decide what to do with Derricka. He caters to his immediate needsi. Foodii. Warmthiii. Wateriv. Sleepb. He calls his mother and has her come and get him and take care of himc. Derrick comes in the next day and begins to talk to Dr. Halgin now that he has significantly calmed down.d. Dr. Halgin continues to see him for several more
