GHTH 100: Personal WellnessFall 2009, Section 2: T/TR 9:30am-10:45amLecture Hall: ISAT/CS 159Instructor: Ms. Christina RohrbaughE-mail: [email protected] Hours: Mon. 11:30am – 1:30pm, Tues/Thurs. 10:45am – 11:45am, Wed. 12:00pm – 1:00pm Office: HHS 3116I. Course Description:This course is designed to examine lifestyle behaviors that contribute to health promotionand disease prevention. Factors affecting health status will be identified and suggestions will be made as to how behaviors, self-care and daily decisions contribute to health and wellness. II. Course Objectives:At the completion of this course, students will be able to demonstrate the following: - Understand the dimensions of wellness, the various factors affecting each dimension, and how dimensions are interrelated.- Understand the relationship between personal behaviors and lifelong health and wellness.- Recognize an individual’s level of health and wellness and understand how these levels impact quality of life and development.- Identify and implement strategies that improve an individual’s wellness.- Use reputable sources to learn about and evaluate current societal trends in health and social behavior. - Acquire knowledge about health and utilize it to help make meaningful decisions throughout life. - Acquire a conceptual understanding, and vocabulary sufficient to communicate with the allied health community about health needs. - Recognize and seek solutions for common health problems. III. Required Textbook, CPS Clicker and Wellness Passport Verification Forms:- Donatelle, R. (2008). Access to Health, 10th edition, Pearson – Benjamin Cummings- CPS Clicker. This radio frequency remote will be used for survey participation and taking exams. Purchase these at the JMU Bookstore. Do Not register them on E-instruction, use the directions on Blackboard under Course information to register them through Blackboard. The clickers use AA batteries, make sure you have extras.1- Wellness Passport Verification Forms are located on Blackboard under Assignments. These forms must be printed off on cardstock and taken to passportevents to be stamped.IV. JMU Honor Code:- Students should observe complete honesty in all academic matters. See the JMU Honor Council web page for a complete description of violations and actions.V. Course Evaluation:- Examinations - There will be a total of four examinations; all four are chapter tests, including the final which is not-comprehensive. Exams are given during regular class time and will use Clicker Technology for submitting answers (points will be deducted for not using clickers). No test make-ups will be given unless there are extenuating circumstances. The student must contact the instructor priorto when the exam is given in class. If the instructor has not been contacted, then the student will forfeit the right to take the exam and will receive a 0.- Pre- and Post-Assess Yourself Survey - You will be required to complete the survey twice; once at the beginning of the semester (Pre-test) and once at the end (Post-test). The survey is found on pages 29-34 in your textbook. You will need your scores from the Pre-Test to answer questions on the Post-Test so make a copy of them to keep for your records. You will also need to answer questions about your scores and turn this in as a one to two page handout which must be typed. The questions that you must answer for this assignment will be posted on Blackboard under Assignments. - Wellness Passport Assignment - This is a series of 5 assignments designed for your participation in activities from a pre-selected list of campus-wide events. Instructions are found under Assignments on Blackboard. These experiences are based on five of the six areas of wellness Emotional, Intellectual, Occupational, Social, and Spiritual/Environmental (this counts as one dimension). The Wellness Passport verification form that is posted on Blackboard must be used. The form must be photocopied onto white cardstock in order to receive full credit. If the form is not photocopied onto cardstock before receiving the passport stamp, 5 points will be deducted from each form. Copies on cardstock can be purchased in the HHSB copy center in room. Blank cardstock pages can be purchased as well. The dates and description of the activities and events can be found at the following website address http://wellness.healthsci.jmu.edu/ under Events Calendar. This on-going semester-long assignment requires you to continuously check the website for posted events. KEEP TRACK OF THE PASSPORT EVENTS THAT YOU COMPLETE.2 Attend five different Wellness Passport events Each Wellness Passport form must be printed on white cardstock Each Wellness Passport form must be stamped to verify attendance- Individual Physical Activity Log Assignment: 30 hours of physical fitness recorded by you! The excel spreadsheet for this assignment can be found on the Blackboard website. All 30 hours can be completed at UREC. Only 15 hours MUST be completed at UREC, 15 hours can come from varsity sports practice, and/or recreation and fitness activities that you complete outside UREC. Can only log 3 hours per week, and 1 hour per day of activity. Keep a hard copy of your log.VI. Grading Scale:A = 450 – 500*B = 400 - 449C = 350 - 399D = 300 - 349F = 299 or below*+ and – scores will be given at the discretion of the instructor. 3VIII. Class Policies- Email communication. Identify your first and last name and your class section.- Special Assistance. The Office of Disability Services provides academic accommodations for students. If you have questions or would like to schedule an appointment call 568-6705.- Attendance. Attendance is expected and will be taken during each class period. Students are expected to attend class regularly and on time. Signing the attendance sheet indicates that you were present for the entire class. If you sign the attendance sheet and leave before class is dismissed, you have violated the JMU Honor Code. Sleeping in class will not be tolerated. A seating chart will be enforced after the second week of class.If you have 0 absences, you will be awarded 10 bonus
