(2009) Access to Health Green Edition, Benjamin CummingsCourse Description: This course is designed to introduce students to the lifestyle behaviors that will contribute to health promotion and disease prevention. Factors affecting health status will be identified and suggestions will be made as to how health-related behaviors, self-care, and individual decisions contribute to health and wellness.Course objectives: At the completion of the course, students will be able to demonstrate through testing, discussion, and personal experience, the following:- Identification of the dimensions of wellness, as well as the impact of heredity; lifestyle, and environment on well-being.- Understand the dimensions of wellness, the various factors affecting each dimension, and how dimensions are interrelated. - Understand the relationship between personal behaviors and lifelong health and wellness.- Recognize an individual’s level of health and wellness and understand how these levels impact quality of life.- Identify and implement strategies that improve an individual’s wellness.- Knowledge about health and its utilization in making meaningful decisions throughout the life cycle. - Examination of personal behaviors related to health and wellness.- Assessment and interpretation of one’s state of overall wellness, as well as the development and implementation of a personal wellness plan. - The ability to use reputable resources to learn about and evaluate current societal trends in health and social behavior.- The ability to communicate, at an acceptable level, about health needs.- The ability to work with others to solve common health problems.JMU Honor Code:Students shall behave in an honest manner in all academic matters. See the JMU Honor Council web page for a complete description of violations and actions.- Accommodation Requests: o The Director of Disability Services (x6705, Wilson 107) is JMU’s designated ADA compliance official. Any student who, because of a disabling condition, may require some special arrangements in order to meet course requirements, should contact Mr. Hedrick as soon as possible. An initial prerequisite to receiving any necessary accommodations is completed documentation on file with the Office of Disability Services.- Class Conduct:o In order for all students to benefit, everyone must behave in a mature, professional manner. All cell phones and pages must be turned to vibrate or silent during class time. If a student feels it necessary to answer a phone or open a text during class time, that student must exit the classroom to do so. The instructor of the class will determine if a student’s pattern of responding to a phone or text is becoming problematic. If laptop use is desired, the student must secure permission from the instructor before using a laptop during class time. If permission is granted, the laptop must be used for class-related activities ONLY.  All pre-class activities must cease by the official class start time. Talking, other than asking a question, during the class period is considered disruptive. Additional descriptions of disruptive behavior included but are not limited to:- Eating in class- Monopolizing discussions- Failure to respect others- Chatting- Constant questioning- Overt inattentiveness- Sleeping- Reading the newspaper- Using laptops for non-class related activities- Using phones- Texting on phones- Doing work not related to class- Creating excessive noise- Leaving class early- Poor personal hygiene Additional rules and regulations associated with student conduct are detailed in the Student Handbook. Failure to abide by all of these rules and regulation s may lead to a variety of disciplinary actions including but not limited to:a) A failing gradeb) Removal from this classc) A summons to appear before the Judiciary Councild) Being placed on probatione) And/or expulsion from the universityCOURSE EVALUATION:- Examinations (4 worth 100 points each)o There will be four exams given over the course of the semester, specific dates are provided on the tentative course outline. Exam questions will cover text and lecture material. The final exam will not be comprehensive.o Students must be present for all exams unless prior arrangements have been made withthe instructor. Make-up exams may be scheduled with the instructor. Make-up exams should be arranged in the week following the originally scheduled exam. After 2 weeks,the instructor may refuse to allow a make-up exam or provide an alternative exam in order to for the exam results to be reviewed with the class.o Leaving an examination without permission from the instructor will result in immediate failure.o If it is determined that an exam cannot be given or an assignment cannot be turned in due to inclement weather or any other University emergency, the exam or class assignment will be due the following class session.o If the final exam cannot be given for the above stated reasons, the final exam will be given on the University make-up day (check the JMU Homepage for information). If it is determined that a final exam cannot be given for above state reasons, the instructor will assign final grades based on all student work completed prior to the final exam date. - Pre- and Post-Wellness Assessments (2 worth 10 points each, total 20 points) o Due September 11, 2009 and November 20, 2009o All GHTH 100 students must complete a pre- and post-wellness assignment, which will be further discussed in class and are described in the course manual.- Wellness Passport Assignment (50 points) 3 due October 16, 2009; 2 due November 30, 2009. Assignments may be turned in during theprevious class period but are due no later than noon on the due date at my office. o This assignment provides students with the opportunity to experience health enhancing activities, programs, and services offered to all JMU students, as well as programs designed specifically for GHTH 100 students.o The assignment is a combination of three responsibilities:1. Attendance at five different Wellness Passport events.2. Having
